How to Fill Out California Form FL-150 Line by Line

Judicial Council Form FL-150 (Income and Expense Declaration) is the foundational financial disclosure form required in every California family law proceeding involving child support, spousal support, or attorney fee requests.

1. General Information & Employment (Page 1)

Item 1 requires your employment information. If you are self-employed or an independent contractor, list your business name and check "Self-employed". Item 1d requires you to attach copies of your pay stubs for the last two months and your most recent federal and state income tax returns with all schedules (redact Social Security numbers for privacy under Cal. Rules of Court, Rule 1.201).

2. Gross Monthly Income Normalization (Page 2)

Item 5 requires you to state your average monthly gross income. The most common error is multiplying a bi-weekly paycheck by 2 (which accounts for only 24 paychecks per year instead of 26). Use the statutory formula: (Gross Paycheck × 26) ÷ 12.

If you receive commission, overtime, or annual bonuses (Item 5e & 5f), list the 12-month average or attach an addendum detailing monthly fluctuations.

3. Monthly Living Expenses (Page 3)

Item 13 requires an accurate listing of your current average monthly expenses. If you are sharing housing expenses with another person, list your actual out-of-pocket contribution. Expenses should reflect the reality of your current living situation, not aspirational budgets.

4. Child Support Information & Custody (Page 4)

Page 4 is required whenever minor children are involved. You must specify the number of children and the approximate percentage of time each child spends with you (timeshare percentage). This figure directly feeds the California statutory Dissomaster / X-Spouse guideline child support calculation.
